Oral cancer screening is a quick clinical exam that looks for early signs of cancerous or pre-cancerous changes in the mouth, throat, lips, and surrounding tissues. The American Dental Association on MouthHealthy.org reports that more than 50,000 Americans are diagnosed with oral or oropharyngeal cancer each year, and survival rates jump dramatically when the cancer is caught at an early stage.

The exam itself is simple. Dr. Youngblood looks for tissue changes you would not notice on your own — color shifts, lumps, persistent sores, or asymmetry — that warrant follow-up. Most findings turn out to be benign, but the few that do not are exactly the cases where early detection saves a life.

What Happens During an Oral Cancer Screening?

The screening is built into your routine checkup and follows three steps:

1. Visual Examination

Dr. Youngblood uses a bright light and small dental mirror to inspect:

  • Lips (inside and outside, including lip line for sun damage)
  • Gums and inside of the cheeks
  • Tongue (top, bottom, sides — pulling the tongue gently to view the base)
  • Floor of the mouth under the tongue
  • Hard and soft palate
  • Tonsils and back of the throat
  • He looks for white patches (leukoplakia), red patches (erythroplakia), persistent sores, lumps, asymmetry, or unusual texture.

    2. Tactile (Palpation) Examination

    Using gloved fingers, Dr. Youngblood gently feels:

  • The neck and under-the-jaw lymph nodes (checking for swelling or hardness)
  • The floor of the mouth and base of the tongue
  • The cheeks and lips for any nodules or hardened tissue
  • Many early oral cancers are felt before they are seen.

    3. Optional Adjunct Screening (VELscope or Similar)

    For patients with elevated risk — tobacco use, heavy alcohol consumption, prior HPV, family history of head/neck cancer, or noticeable risk factors — Dr. Youngblood may use a fluorescence visualization device (such as VELscope) to highlight abnormal tissue at a cellular level before symptoms appear. This is an optional add-on, typically $25 to $50, and not always covered by insurance.

    The complete screening, including the optional adjunct, takes about 5 minutes. The standard visual + tactile screening takes 2 to 3 minutes.

    What Are the Risk Factors for Oral Cancer?

    Some risk factors significantly raise your chance of developing oral or oropharyngeal cancer:

  • Tobacco use — cigarettes, cigars, pipes, chewing tobacco, snuff (the largest single risk factor)
  • Heavy alcohol consumption — risk multiplies when combined with tobacco
  • Human papillomavirus (HPV) — especially HPV-16, linked to a growing share of oropharyngeal cancers (back of the throat, tonsils, base of the tongue)
  • Sun exposure — especially for cancers of the lip
  • Age — most diagnoses occur after age 40, though HPV-related cases are appearing in younger patients
  • Sex — men are diagnosed about twice as often as women

    Who Should Be Screened — and How Often?

    What Happens If We Find Something During Your Screening?

    The vast majority of screenings come back normal. If we identify an area of concern, here is exactly what happens:

    Step 1: We show you and explain. Using our intraoral camera, we show you the area on a screen and explain why it warrants attention. Many findings turn out to be benign — canker sores, irritation from a rough tooth edge, or harmless tissue variation.

    Step 2: Watch-and-wait or referral for biopsy. For minor findings, we may photograph the area and ask you to return in 2 weeks to see if it has resolved on its own. If a finding is clearly suspicious or persists, we refer you to an oral surgeon or periodontist for a biopsy. The American Association of Oral and Maxillofacial Surgeons (AAOMS) recommends tissue biopsy as the definitive diagnostic step for any suspicious oral lesion. The biopsy itself takes 10 to 15 minutes under local anesthesia.

    Step 3: Biopsy results. The pathology lab typically returns results within 7 to 10 business days. We contact you directly with the results and explain what they mean in plain language.

Cancers that develop in deeper throat tissues may not be visible without specialized imaging. If you have persistent throat pain, difficulty swallowing, or a lump in your neck, let us know immediately.

    What does it mean if you find something during my screening?

    Most findings are benign — canker sores, irritation from a sharp tooth, or normal tissue variation. If we see something concerning, we will photograph it and either watch it for two weeks or refer you for a biopsy with a specialist. Most biopsied lesions are also benign — but for the small number that are not, early detection makes all the difference in treatment success.

    How soon can I return to work after an oral cancer biopsy?

    Most patients return to work the same day or the next day. The biopsy uses local anesthetic and takes 10 to 15 minutes. You may have mild soreness and slight swelling at the site for 2 to 3 days, easily managed with ibuprofen. Avoid hot liquids, alcohol, and very crunchy foods for the first 24 hours.

    Is the VELscope or fluorescence screening worth the additional cost?

    For higher-risk patients (tobacco users, heavy drinkers, prior HPV, family history), the additional $25 to $50 for VELscope can highlight abnormal tissue earlier than visual exam alone.
